The Resident Care Coordinator will work under the Director of Assisted Living to maintain the smooth function of the 58 unit Assisted Living and Memory Care departments in accordance with all internal policies, procedures, and state and federal laws.
An ideal candidate will have a college degree with education in the area of gerontology, dementia care, recreation therapy, social work or equivalent professional experience. The Resident Care Coordinator will be a proactive, enthusiastic contributor to a dynamic team. The position requires good human relation skills and the ability to monitor and direct the activities of others; the ability to enlist cooperation of unit personnel and staff; the ability to maintain good, professional relationships with physicians and relate well to residents and families.
DUTIES
This position will report to the Director of Assisted Living and is clinical and supervisory in nature. The position requires respectful interaction with all residents, family members, visitors and coworkers. In the performance of duties, the Resident Care Coordinator will perform the following functions:
- Contribute to continuity of care with effective communication and coordination of daily services within the assisted living facility.
- May participate in routine activities of resident care including activities of daily living and general assistance.
- Know all residents in the departments by name. Assist with new resident move-ins and establish personal contact with new admissions and resident families as soon as possible.
- Knowledge of the individualized care plan for residents and provide support to residents according to their care plan.
- Contribute to care planning by providing specific observations of the residents' needs and preferences.
- Fully understand and prepared to train others on all aspects of residents' rights.
- Responsible for promptly reporting incidents or evidence of abuse or violations of residents' rights.
- Complete all required documentation to ensure maintenance of resident care information.
- Monitor assignment sheets for timely completion and that all required documentation has been achieved.
- Perform regular rounds to observe residents and to determine if needs are being met.
- Participate in orientation for new members of the department to ensure that all employees are knowledgeable of the goals, policies and procedures for the department.
- Assist with supervision of the assisted living staff including caregivers and medication technicians.
- Assure the safety and security of residents through the proper use of equipment, safety devices and following fire, safety and disaster procedures.
- Ensure the department is tour ready at all times and exemplify facility dress code.
- Participate in ongoing in-service education.
- Handle special projects as assigned by Director of Assisted Living.
QUALIFICATIONS
Our ideal candidate will have a love for seniors and partner with our team to ensure their success. Two (2) or more years of experience with resident care in an assisted living or memory care environment is preferred and knowledge of dementia-specific care is mandatory. Experience supervising and overseeing others is required. Work experience should include basic medical terminology or previous medication pass experience.
Education: Education in gerontology, dementia care, recreation therapy, social work or extensive equivalent professional experience preferred.
Reasoning Skills: Understand and conceptualize concepts as well as demonstrate ability to make decisions in stressful situations.
Oral/Written Communication Skills: Be able to explain ideas and to communicate complex ideas to a wide audience (employees, residents, family members, etc.), both in writing and verbally.
